Community Health Assessment
Montgomery County's Department of Health & Human Services Office of Public Health will be launching the first Community Health Assessment to better understand the community's health status, concerns, and priorities. The purpose of this survey is to identify and prioritize the most important health issues affecting Montgomery County communities and opportunities to make improvements. Other purposes of this survey include;
- Describe a community’s health status, needs, and the factors that contribute to health
- Identify inequities experienced among subpopulations within the community
- Identify the assets and resources that support health and wellness
This survey is anonymous. Community Health Assessments typically includes information about; health status of a community (illnesses/diseases that may be common or particularly concerning), quality of life, health behaviors, factors that contribute to health, such as socioeconomic and environmental conditions affecting the community, community assets and resources, and community stories and perspectives.

The CHA is open to people who live, work, and learn in Montgomery County to share their perspectives about, and experiences with, health and wellness. The results of the survey are one part of painting the picture of the community’s health and wellness needs and priorities.
The data collected will be analyzed, summarized, and shared back with the community. Using what is learned through this process, the County will work with community partners to develop a collaborative, community-wide, transparent and long-term Community Health Improvement Plan. This plan will be designed to help meet the most critical needs and priorities of all Montgomery County residents, especially those underserved.
